Health Information
 | Welcome to the Health Information Department
The Health Information Department is responsible for medical records, transcription, coding, medical staff credentialing and utilization review. Leslie Janssen, Director of Health Information, also serves as the HIPAA privacy officer for Gonzales Healthcare Systems and the performance improvement coordinator. |
If you have a question about your health information, please call us at (830) 672-7581 ext. 438. The Health Information Department is open Monday – Friday, excluding Holidays, 8:00 a.m. – 5:00 p.m.
If you need a certified copy of a birth certificate, contact the County Clerk’s office in Gonzales or the Texas Department of Health Bureau of Vital Statistics in Austin. |
