For over 20 years, Memorial Hospital Home Health Agency has served the residence of Gonzales County and surrounding areas by providing comprehensive in-home health and supportive services. Our care giving team offers services ranging from basic assistance with daily living to advanced medical treatment. We offer traditional home health services that include: nursing, social services, physical and occupational therapy. In addition, we offer several types of home-maker programs to meet non-nursing needs in the home.

Traditional Home Health Services

Registered nurses supervise the physician ordered plan of care to meet the health goals for the patient. This traditional “Home Health” is the type most familiar to our community. It’s primary goals are to teach the homebound patient and their family to manage health conditions safely and prevent hospitalization.

PRIMARY DIAGNOSES SERVED
There are five categories most commonly treated as traditional home health patients. (Services are not exclusively limited to these categories.)

THERAPY SERVICES (PT, OT, SPEECH): Includes joint replacement home therapy, stroke, gait abnormalities, weakness and fall prevention/home safety teaching.

CARDIAC PATIENTS: Includes skilled nursing assessments for high blood pressure, congestive heart failure, atria fibrillation, and other heart diseases.

RESPIRATORY PATIENTS: Includes patients with chronic obstructive pulmonary disease, convalescence from pneumonia, and other oxygen teaching.

DIABETIC PATIENTS: Includes complications from diabetes such as wound care, home glucose monitoring, and renal complications.

UROLOGICAL PATIENTS: Includes teaching about signs/symptoms of problematic urinary tract infections, and Foley catheter care for homebound patients




Caregiver Home Health Services

Home care attendants perform light household duties, such as laundry, meal preparation, general housekeeping, and shopping. These programs include:

CBA: A Medicaid program which provides a community based alternative to nursing home placement.

PRIMARY HOME CARE: A Medicaid program which provides basic home care activities including the Family Care Program and the Frail and Elderly Primary Home Care Program.

GOLDEN CRESCENT: Homemaker services for limited support of house cleaning tasks.

BATH & BREAKFAST: A self pay non-skilled program.
