When the physician gives you an order to receive outpatient testing or schedules you for testing, please report to the Admissions Department which is located in the out-patient lobby of Gonzales Healthcare Systems. The admissions office is open Monday through Friday 7:30 a.m. – 5:00 p.m. Please arrive at least 15 minutes prior to your scheduled appointment. This time allows us to finish serving customers who arrive before you and to give you the time necessary to complete all required paperwork prior to your service.
Pre-registration expedites your check-in process on your appointment day. There may be a few additional questions we need to clarify and we will ask you to sign a consent form. Please bring the physician’s order and all insurance cards at the time of admission. If you have any questions, please contact our admissions department at (830) 672-7581 ext. 2600.
When coming for outpatient testing you must bring your doctor’s written order for treatment. The Federal Government states that a complete physician’s order must include the following elements:
- Patient’s Full Name
- One additional identifier (i.e., date of birth or social security number)
- Service Ordered
- Diagnosis or Signs and Symptoms
- Physician Signature
- Date of Order
Medicare requires us to verify medical necessity for certain outpatient testing. If the testing does not meet Medicare’s guidelines, you will be asked to sign an Advanced Beneficiary Notice stating that you will be financially responsible for the service.
